ABOUT THE PRACTICE

Our mission is simple - to make you feel and stay healthy! 

How is Vermont Integrative MD different than your typical primary care practice?

  • Initial visits with Dr. Bazel are 1 hour to 1.5 hours depending on the time needed to discuss your health history and current problems. 

  • Follow-up visits are 30 minutes, or longer if needed. 

 

This gives your doctor time to discuss lifestyle interventions in detail and come up with an individualized treatment plan that works for you.

The focus is a collaboration between you and your physician.  Your doctor will work with you to come up with a plan that best fits your needs and your health care goals. 

Furthermore, there will be no long waits to see your doctor.  If you are sick, Dr. Bazel will schedule you for a visit within 1 to 2 days in the office.   

Every appointment will be with Dr. Bazel, guaranteeing that you will have continuity with your doctor each and every visit.

 

Phone and email access is available for urgent needs after hours during the week and on weekends.  Dr. Bazel also sees patients at assisted living facilities or can drive to your work place or home for an additional fee if needed.

SERVICES

  • General wellness exams or Physicals

  • Gynecologic exams

  • Management of acute and chronic conditions

  • Well child exams

  • Counseling on lifestyle interventions (nutrition and exercise)

  • Teaching of Mind-Body techniques for various chronic conditions (insomnia, stress, etc.)

  • Coordination with specialists and your other doctors if needed for your care

  • Vaccinations

Lab services are offered at nearby UVM Medical center labs.

Limited in-house lab testing is offered including urine analysis, rapid strep testing, rapid flu testing.

FEES

PRACTICE

MEMBERSHIP

The membership fee includes an annual physical and any additional visits needed to provide ongoing care for medical problems as well as acute care visits. 


There are no insurance co-pays, deductibles or co-insurance fees.

Individual rates:

 

  • $2,500 Annual Membership fee 

  • Can be paid annually or quarterly

 

Families:

 

  • Each child with a parent who is a member: $500/ year per child

Dr. Bazel can see patients at Assisted Living facilities if needed for an additional $500 fee per year to include traveling time to see patients on-site.
 

INTEGRATIVE MEDICINE CONSULTATIONS

Do you have a primary care doctor, but want an integrative medicine consultation?


An integrative medicine consultation can be helpful for ongoing chronic issues such as diabetes, irritable bowel syndrome, cancer and many other conditions, but can also be useful if you want to learn about extensive preventive measures to keep you healthy.


You can be referred for a consultation by your doctor or through a self-referral.

Rates:

  • $350 per hour for an initial consultation (typically 1.5 - 2 hours) and for follow-up visits.

A superbill can be provided for you to submit to your health insurance for possible reimbursement.

Lyme Disease Consultations

If you are interested in a consultation for Lyme disease or to test for Lyme disease, please call the office to schedule an appointment.

Cost of the appointment is the rate of an integrative medicine consultation $250/ hour plus any additional fees for labwork that may be out of network with your insurance
